This extraordinary efficacy is precisely why global demand for Mounjaro has been so intense - and why supply has consistently struggled to keep pace.
Why Has Mounjaro Been Hard to Get in Ireland?
When tirzepatide was first approved in the US in 2022, demand immediately outstripped Eli Lilly's manufacturing capacity. The FDA placed it on its official drug shortage list in December 2022 - a designation that persisted in various forms for nearly two years. At the height of the shortage in early 2024, four out of six dose strengths were listed as having limited availability nationally.
The reasons were structural. Tirzepatide is a complex peptide molecule - manufacturing it at scale requires highly specialised facilities and cannot simply be ramped up quickly. Eli Lilly's own manufacturing head acknowledged publicly that patients would need to wait years, not months, for new plant capacity. Meanwhile, pharmacies began bulk-ordering to secure stock, which distorted demand signals further.
In Ireland specifically, the problem was compounded because Mounjaro was being prescribed for both weight management and type 2 diabetes - competing for limited supply across two patient populations simultaneously.
The situation has substantially improved. In October 2024, the FDA confirmed the tirzepatide shortage was resolved, following major investment by Eli Lilly - including a confirmed $1.8 billion expansion of its manufacturing facilities in Limerick and Kinsale, Ireland. Supply in 2026 is meaningfully more stable than 2023-2024.
However - intermittent local stock variations still occur, particularly at higher maintenance doses. Not every pharmacy stocks every pen strength, and some doses sell out faster than others. This is why the pharmacy network your provider works with matters as much as the prescription itself.
The Counterfeit Problem: A Serious and Escalating Risk in Ireland

In October 2025, the HPRA issued one of its most serious warnings. Counterfeit Mounjaro pens were seized in Ireland that contained insulin instead of tirzepatide. Laboratory testing confirmed that two pens contained insulin - a substance that, if injected by someone without diabetes, can cause severe hypoglycaemia, seizures, loss of consciousness, and death if untreated. The pens were visually indistinguishable from genuine Mounjaro.
HPRA WARNING - DO NOT BUY MOUNJARO FROM UNREGULATED SOURCES
The HPRA confirms there are no authorised generic versions of tirzepatide and no authorised tablet or powder form. Any Mounjaro sold as a tablet, powder, or vial - or sold without a valid prescription from an unregistered channel - is illegal and potentially dangerous. Social media advertisements, beauty clinics, and unlicensed websites are not safe sources. No exceptions.
How to make sure you are getting genuine Mounjaro
The following are non-negotiable when accessing Mounjaro safely in Ireland:

Who Is Eligible for Mounjaro in Ireland?
Eligibility follows the EMA Summary of Product Characteristics for Mounjaro. You need either:
- A BMI of 30 or above, or
- A BMI of 27-29.9 alongside at least one weight-related health condition - such as type 2 diabetes, high blood pressure, high cholesterol, obstructive sleep apnoea, or cardiovascular disease.
Mounjaro is not suitable if you have a personal or family history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2). It should not be used during pregnancy.
