Is Mounjaro Covered by the HSE or Medical Card in Ireland?

This is one of the most common questions we receive, and it deserves a straight answer.

No. Mounjaro is not currently covered by the HSE, the Medical Card (GMS scheme), or the Drugs Payment Scheme (DPS) for weight management in Ireland. As of mid-2026, patients who use Mounjaro for weight loss pay privately for both the medication and the clinical support.

That is the headline, but there is more to the story, and it is worth understanding what is under review, what limited options do exist within the public system, and how to manage the cost if you are paying out of pocket.

The Current Reimbursement Picture

The Irish system for funding prescription medications runs through the HSE, with decisions informed by the National Centre for Pharmacoeconomics (NCPE). Before a medication can be added to the reimbursement list, it goes through a health technology assessment (HTA) that evaluates both clinical effectiveness and cost-effectiveness.

Here is where each weight loss medication stands as of mid-2026:

The only weight loss medication with any level of public funding is Saxenda, and access is extremely limited. The Managed Access Protocol requires patients to be adults with a BMI of 35 or above who also have prediabetes and elevated cardiovascular risk. Even then, reimbursement is granted in two phases - you must demonstrate at least 5% weight loss after 12 weeks on the full dose to continue, and total coverage is limited to two years.

For the vast majority of patients seeking medical weight management in Ireland, private payment is the reality right now.

What About Mounjaro for Diabetes?

There is an important distinction here. Mounjaro (tirzepatide) has a dual licence - it is approved for both type 2 diabetes and weight management. The reimbursement question depends on which condition it is being prescribed for.

For type 2 diabetes, the NCPE process and reimbursement status may differ. If you have type 2 diabetes and your doctor is considering Mounjaro as part of your diabetes management, the funding pathway is separate from the weight management question. Discuss this directly with your GP or endocrinologist.

Will Mounjaro Be Covered in the Future?

Whilst it remains to be seen, the NCPE is currently conducting a health technology assessment for Mounjaro, and some analysts consider it a strong candidate for reimbursement given:

  • Its different active ingredient to Wegovy (tirzepatide vs semaglutide), which means it is not a "me too" product
  • The greater average weight loss demonstrated in clinical trials (up to 22.5% in SURMOUNT-1)
  • Growing international evidence of cost-effectiveness when factoring in reduced downstream healthcare costs for obesity-related conditions

However, cost remains the central issue. The Irish Examiner reported in late 2025 that the HSE had been told the projected cost of funding Mounjaro for weight loss was substantial. The gap between what the HSE is willing to pay and what Eli Lilly is asking is the core negotiation.

No timeline has been confirmed for a final decision. If reimbursement is approved, it would significantly change the affordability landscape for Irish patients. But it is not something to plan around right now.

How to Manage the Cost Privately

If you are paying out of pocket - which most patients currently are - there are practical ways to bring the cost down.

This may change as medical weight management becomes more established, but for now, insurance is not a reliable pathway to reducing your Mounjaro costs.
