Wegovy Pill Dosing Schedule: What Irish Patients Need to Know

Now that the European Commission has approved the Wegovy pill for weight management across the EU, one of the most common questions we are hearing is about the dosing. How often do you take it? What dose do you start on? Is it complicated?
The answer is that the pill follows a similar logic to the injection - you start low and build up gradually - but the daily routine is different enough that it is worth understanding properly before you begin.
This guide covers the full dosing schedule based on the OASIS clinical trial programme and the approved product information. Irish pharmacies are expected to begin stocking the pill in late 2026 to early 2027, so this is information you will want to have ahead of time.
The Titration Schedule
Like the injectable Wegovy, the pill uses a stepped dose escalation. You do not start on the full therapeutic dose. The titration is designed to let your digestive system adapt gradually, reducing the severity of common side effects like nausea.
Step 1 · 3mg · 4 weeks
Starting dose. Allows the body to begin adapting to oral semaglutide.
Step 2 · 7mg · 4 weeks
First increase. Appetite effects become more noticeable.
Step 3 · 14mg · 4 weeks
Intermediate step. Most patients begin to see meaningful appetite changes.
Step 4 · 25mg · Ongoing (maintenance)
Full therapeutic dose. This is where the clinical trial results were achieved.
The total titration from starting dose to maintenance takes approximately 12 weeks. Your prescribing doctor may extend any step if you are experiencing significant side effects. The pace should always prioritise tolerability - there is no benefit to rushing through the titration if it makes the experience miserable.
A prescription for the Wegovy pill is not guaranteed. Eligibility is determined by an IMC-registered doctor after a full clinical assessment, and the same BMI and health criteria apply as for the injectable version.
The Daily Routine
This is where the pill differs most from the injection, and it is the part that matters for your day-to-day life.
When to take it: First thing in the morning, on an empty stomach, after fasting for at least 8 hours (overnight counts).
How to take it: Swallow the tablet whole with no more than 120ml (about half a glass) of plain water. Do not crush, chew, or split the tablet.
After taking it: Wait at least 30 minutes before eating breakfast, drinking anything else (including tea or coffee), or taking any other oral medications.
Why the 30-minute window matters: Oral semaglutide is absorbed through the lining of your stomach. Food, drinks, and other medications can interfere with that absorption, reducing how much of the drug actually gets into your system. If you eat too soon, you may not get the full benefit of the dose.
For some people this morning routine will feel natural - wake up, take the pill, shower, get dressed, then eat breakfast. For others, especially those who reach for the kettle before their eyes are fully open, it will take a week or two of adjustment.

Neither format is objectively better. The pill removes the needle entirely but adds a daily commitment and a morning routine. The injection is once a week with no food timing requirements but involves a needle. Many patients will have a clear preference, and that preference is valid.

What About Side Effects on the Pill?
The side-effect profile is consistent with injectable semaglutide. The most common effects are gastrointestinal - nausea, constipation, diarrhoea - and they follow the same general pattern: appearing during dose increases and easing as the body adjusts.
In the OASIS 4 trial, the safety profile of oral semaglutide 25mg was consistent with what has been seen in prior semaglutide trials across both oral and injectable forms. There were no new safety signals.
One practical note: if you experience nausea after taking the pill, the same dietary strategies that help with the injection apply here. Eat bland, easy-to-digest food when you break your 30-minute fast. Avoid greasy or heavy breakfasts on dose-increase days. Sip water steadily through the morning.
Can You Switch from Injection to Pill?
This is a clinical question that will need to be discussed with your doctor when the pill becomes available in Ireland. The two formats contain the same active ingredient, but switching mid-treatment involves decisions around dose equivalence and the transition between weekly and daily dosing.
Your prescribing doctor will manage this transition if it makes sense for your situation. It is not a decision to make on your own or without medical input.
