How to Inject Mounjaro: A Step-by-Step Guide for Irish Patients

The idea of injecting yourself sounds worse than the reality. That is genuinely true, and nearly every patient we work with says the same thing after their first time - that they had built it up in their head far more than the experience warranted.

Mounjaro uses a KwikPen, which is a prefilled, single-use auto-injector. There is no drawing up medication from a vial, no mixing, no assembly. You uncap it, press it against your skin, hold a button, and the pen does everything else. The needle is very fine, and most patients describe the sensation as a brief pinch that lasts about five seconds, if they feel it at all.

This guide walks through the full process from taking the pen out of the fridge to disposing of it afterwards. If you have already been shown how to inject by your doctor or pharmacist, this is a reference to come back to if you need a refresher. If you have not started yet and want to know what is involved, this will take the mystery out of it.

A reminder that Mounjaro is a prescription-only medicine, and your doctor or pharmacist will provide personalised instructions when your treatment begins. Always follow the guidance specific to your prescription and the patient information leaflet included with your pen.

Before You Inject: Preparation

  • Take the pen out of the fridge 30 minutes beforehand: Mounjaro pens should be stored in the fridge (2-8 degrees C) before first use. Injecting cold medication can cause more discomfort at the injection site, so letting the pen reach room temperature makes the process more comfortable. Do not use a microwave, hot water, or any other heat source to warm it up - just leave it on the counter.

  • Check the pen: Look at the medication through the viewing window on the pen. It should be clear and colourless. If it looks cloudy, discoloured, or contains particles, do not use it - contact your pharmacy for a replacement.

  • Check the expiry date: printed on the pen and the outer carton. Do not use an expired pen.

  • Wash your hands with soap and water before handling the pen.

  • Choose your injection site: You have three options, and you should rotate between them each week.

The Injection: Step by Step

This takes less than a minute once you have done it a couple of times. The first time might feel slower because everything is new, and that is completely fine.

Remove the grey base cap by pulling it straight off. This exposes the needle, which is covered by a safety shield and is not visible until the pen is pressed against your skin. Do not touch the needle guard or try to look at the needle - there is no need to.

Place the flat grey base of the pen firmly against your chosen injection site. Hold the pen at a 90-degree angle to your skin (straight on, not at an angle).

Press and hold the injection button (the purple button on the top of the pen). You will hear a click, which means the injection has started. Keep holding the button and keep the pen pressed against your skin.

Hold for about 10 seconds, or until you hear a second click and the grey plunger becomes visible in the window. This confirms the full dose has been delivered.

Lift the pen straight away from your skin. The needle will retract automatically into the pen body - you will not see or touch the needle at any point during the process.

That is it. The whole injection takes about 10-15 seconds of active time. If you see a small drop of blood or clear liquid at the injection site, that is normal and nothing to worry about - dab it gently with a cotton pad if needed, but do not rub the area.

Rotating Your Injection Site

Using the same spot every week can cause skin irritation, lumps, or changes in how the medication is absorbed. Rotating between sites helps prevent this and keeps the injection comfortable over the long term.

A simple rotation that works well is to cycle through three zones across the week: abdomen one week, right thigh the next, left thigh the next, then back to abdomen. If you have someone who can help, you can include the back of the upper arm in your rotation as well.

Within each zone, vary the exact spot slightly each time. If you injected into the left side of your abdomen last time you used that zone, go to the right side next time. The idea is to avoid hitting the same square centimetre repeatedly.

Storage and Handling

  • Before first use: store in the fridge at 2-8 degrees C. Do not freeze. If a pen has been frozen, do not use it.

  • Once in use: Mounjaro pens can be stored at room temperature (below 30 degrees C) for up to 30 days. After 30 days out of the fridge, the pen should be discarded even if there is medication remaining. Keep it out of direct sunlight and away from heat sources.

  • Travelling: keep the pen in an insulated cooling bag if you are travelling for more than a few hours, especially in warm weather. If you are flying, carry your medication in your hand luggage along with a copy of your prescription or a letter from your doctor confirming it is prescribed treatment. Airport security in Ireland and across the EU are familiar with injectable medications and should not cause any issues, but having documentation avoids unnecessary delays.

What If You Miss a Dose

If you miss your injection day, take it as soon as you remember, as long as it is within 4 days of when you were supposed to inject. Then continue with your normal weekly schedule from the next injection.

If more than 4 days have passed since your missed dose, skip it entirely and take your next dose on your regular injection day. Do not take a double dose to make up for a missed one.

If you find yourself missing doses regularly, that is worth discussing with your clinical team, as consistency is important for getting the best results from treatment.

Disposing of Used Pens

Used Mounjaro pens contain a retracted needle and should not go in your household bin. In Ireland, you can return used pens and needles to your local pharmacy, which will dispose of them safely as clinical waste. Most pharmacies accept sharps returns without charge.

Some local authorities also provide sharps bins or collection points - check with your county council if you prefer that option.

Do not attempt to recap or disassemble a used pen. The needle is retracted and locked inside the pen body after use, and there is no need to access it.

Common Concerns

  • "What if I do it wrong?": The KwikPen is designed to make errors very difficult. If you hear the two clicks and see the plunger in the window, the dose has been delivered correctly. If you are unsure whether the full dose went in (for example, if the pen slipped away from your skin during the injection), do not take a second dose. Contact your clinical team for advice.

  • "What if it hurts?": Most patients are surprised by how little they feel. The needle is 4mm long and extremely fine. If the injection stings, letting the pen warm to room temperature for longer before injecting usually helps. Injecting into an area with more subcutaneous fat (like the abdomen) tends to be less noticeable than leaner areas.

  • "What if I see blood?": A small spot of blood at the injection site is completely normal and just means you nicked a tiny blood vessel near the surface of the skin. Apply gentle pressure with a cotton pad for a few seconds and it will stop. This does not affect the medication or mean you did anything wrong.

  • "What if I am genuinely terrified of needles?": That is valid, and you are not alone. Many patients feel anxious before their first injection and find it manageable once they have done it. If needle fear is a significant barrier for you, talk to your doctor about it - there are strategies that can help, and the Wegovy pill (recently approved in the EU) will offer a needle-free alternative once it reaches Irish pharmacies.
