Wegovy (semaglutide) is a once-weekly injectable prescription medicine used for weight management in adults who are overweight or have obesity. Now available in Ireland up to the 7.2 mg dose, Wegovy is designed to work alongside a reduced-calorie diet and increased physical activity. Generally, treatment can be suitable for patients with a BMI of 30 or above, or a BMI of 27 or above if you have a weight-related health condition such as high blood pressure.

Patient Information: Wegovy

What is Wegovy?

Wegovy is a prescription medicine (active ingredient: semaglutide). It is prescribed for adults to support weight management and weight control, and is also used in the treatment of type 2 diabetes. It works by acting on receptors in the brain that control your appetite, which can make you feel fuller, less hungry, and experience fewer food cravings.
Wegovy is designed to work alongside lifestyle modifications. To achieve the best results, it should be part of a plan that includes a reduced-calorie diet and increased physical activity. Wegovy is available in a range of maintenance doses. Most patients settle on 2.4 mg as their long-term dose. For patients who tolerate 2.4 mg well but need additional weight reduction, the 7.2 mg pen is a higher-strength option now available in Ireland. An independent partner GP will review your health profile and determine the appropriate dose.

Key Information

  • Active Ingredient: Semaglutide
  • Administration: Once-weekly injection you can do yourself
  • Primary Use: Long-term weight management in adults with obesity or overweight with weight-related conditions
  • Dose Range: 0.25 mg to 7.2 mg (once-weekly pre-filled pens)
  • Availability: Pre-filled pens; prescribed after clinical assessment by an independent partner GP

Eligibility criteria for Wegovy in Ireland

BMI Thresholds for Eligibility

Eligibility is primarily determined by your Body Mass Index (BMI), which is a measure of your weight relative to your height:
BMI of 30kg/m² or higher: You are generally eligible for treatment based on your weight alone.
BMI of 27kg/m² to 29.9kg/m²: You can't usually access the medication based on weight alone in this range. You'll need to have at least one weight-related health condition, such as:
  • High blood pressure (hypertension)
  • High cholesterol
  • Obstructive sleep apnoea
  • Prediabetes or Type 2 diabetes
  • Heart disease
A doctor will check your health profile to ensure there are no medical reasons that prevent you from taking it. For example, generally, you won't be suitable for Wegovy if you have:
  • Specific Family History: A history of Medullary Thyroid Carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).
  • Pancreatic or Digestive Issues: A history of severe pancreatitis or severe gastroparesis (where the stomach empties too slowly).
  • Pregnancy: If you are pregnant, planning to become pregnant, or breastfeeding.
  • Conflicting Medications: If you are already taking other GLP-1 medications or certain diabetes treatments.

Wegovy Dosing Titration

Wegovy dosage schedule showing 20-week progression from 0.25mg to 2.4mg

The standard titration path builds slowly from 0.25 mg to 2.4 mg over roughly 16-20 weeks to minimise side effects. 2.4 mg is the maintenance dose for most patients. Adults with a BMI of 30 or more when they started treatment who have been on 2.4 mg for at least 4 weeks may be stepped up to 7.2 mg if their doctor decides it is needed. If there is no further weight improvement at 7.2 mg, the dose is lowered back to 2.4 mg. The 7.2 mg pen is not a starting dose and is not appropriate for everyone.

How much weight can I lose on Wegovy?

In the STEP UP trial, people on Wegovy 7.2 mg lost 20.7% of their body weight on average over 72 weeks, and about one in three lost 25% or more. Results vary from person to person. Source: STEP UP (Wharton et al., Lancet Diabetes Endocrinol 2025) and the Wegovy EU product information, section 5.1; figures are for people who took the medicine as prescribed, alongside diet and exercise changes.

With Bua's programme, the trial average would bring you to 79 kg over 72 weeks, based on the trial average. That's a weight loss of 21 kg. Results vary from person to person.

Projected
Continued

This projection is based on the average results from large-scale clinical trials and is for informational purposes only.

  • ✓Average weight loss: 20.7% of body weight over 72 weeks on the 7.2 mg dose
  • ✓Body composition: around 84% of weight lost is fat mass, with muscle largely preserved
  • ✓Cardiovascular benefits: shown to reduce risk of major cardiovascular events in adults with obesity and heart disease
  • ✓Appetite regulation: helps reduce food cravings and portion sizes
  • ✓Long-term maintenance: sustained results depend on continued treatment alongside lifestyle changes

Individual results vary. These outcomes are based on clinical trial data and should be discussed with your healthcare provider.

How to Administer Wegovy

Before first use, your healthcare provider should show you how to inject it. Always read the full "Instructions for Use" leaflet provided with your medicine.

  1. Prepare: check your pen contains your prescribed dose and the solution is clear and colourless. Always use a new needle for each injection.
  2. Choose your injection site: front of your upper legs, your stomach, or your upper arms.
  3. Rotate your site: you may inject in the same body area each week, but not the exact same spot.
  4. Inject: follow the leaflet to set your dose; press and hold the dose button until the counter shows 0, then slowly count to 6 before removing the needle.
  5. Dispose: unscrew the needle immediately after injection and dispose of it safely. Never store your pen with a needle attached.

Important Note: Every time you inject, select a new location that is at least a finger's width apart from the previous site. Rotating your injection sites is crucial for preventing the development of persistent, hard lumps (fatty nodules), which can form and reduce how well your medication works.

The Potential Side Effects of Wegovy

Like all medicines, Wegovy can cause side effects, although not everybody will experience them. It's important to be aware of what to look for.

Common (may affect more than 1 in 10): headache, nausea, vomiting, diarrhoea, constipation, stomach pain, feeling weak or tired. These usually pass over time as your body adjusts.

The 7.2 mg dose has a safety profile consistent with the 2.4 mg dose. In clinical trials, gastrointestinal side effects were slightly more common at 7.2 mg but remained mostly mild to moderate. Around 3% of patients on 7.2 mg stopped treatment due to GI side effects, compared with 2% on 2.4 mg.

Rare – seek immediate medical attention:

  • Signs of acute pancreatitis (severe, long-lasting stomach pain that may move to the back)
  • Severe allergic reactions (breathing problems, swelling of face/throat, wheezing, fast heartbeat)
  • Bowel obstruction (severe constipation with stomach ache, bloating, vomiting)
